In Somalia, tobacco smoking is legally allowed, with some specific set of regulatory frameworks. Within these frameworks, the sale and use of cigarettes are permitted, and consumers can purchase these products through online platforms or vending machines. Notably, the authorities do not require health warnings on cigarette packaging, nor do they enforce a minimum age for purchase or sale. Additionally, there are no legal restrictions for smoking in public places.
There are no data available on the prevalence of smoking in Somalia. However, the number of current smokers was estimated to be 519,000 individuals as of 2024. In 2021, the country reported smoking-related mortality of 4,998 individuals. Of these deaths, 3,905 were male deaths, and 1,093 were females deaths. Tobacco smoking-related mortalities accounted for 2.1% of all deaths in the country, with 2.93% deaths among males and 1.05% deaths among females. This represents a decrease from 2016, when the total number of smoking-related deaths was 3,868, and the percentage of all deaths attributable to tobacco smoking was 4.53%. Specifically, mortality rate due to tobacco smoking was 5.5% among males in 2016, and this rate was 2.2% among females. These statistics underline the ongoing impact of tobacco smoking on public health in Somalia, despite a decrease in mortality rates over the five-year period.

Tobacco harm reduction in Sub-Saharan Africa
With the number of tobacco users across the continent set to rise by 2025, we look at the current state of tobacco harm reduction in the region, assessing the progress made so far and the obstacles preventing greater adoption of safer nicotine products.
African Region - Increasing Daily Tobacco Prevalence Trends (Male and Female)

This figure, using WHO data, shows the steep gradient of increases in smoking prevalence estimates in specific African countries, where the projected increases (both sexes) in daily smoking are 5 percent and higher.
In countries that have experienced steep falls in smoking prevalence over the years, the graphs have begun to level off, for example in Australia, suggesting there remains a substantial number of people who, for whatever reason, are determined to carry on smoking. The WHO trend data to 2025 predicts only very modest falls in smoking levels in several countries and predicts some rapid increases, mainly in Africa, the Middle East, some parts of Eastern Europe and republics of the former Soviet Union. Some of the more extreme examples include Cameroon (from 14 percent to 43 percent); Republic of Congo (from 14 percent to 48 percent) and Bahrain (from 25 percent to 60 percent).
