Brakuje danych dotyczących rozpowszechnienia palenia tytoniu w Somalii. Niemniej jednak w 2021 roku odnotowano w kraju znaczną liczbę zgonów związanych z paleniem tytoniu – 4 998 osób. Spośród nich 3 905 stanowili mężczyźni, a 1 093 kobiety. Zgony spowodowane paleniem tytoniu stanowiły 2,1% wszystkich zgonów w kraju, w tym 2,93% wśród mężczyzn oraz 1,05% wśród kobiet. To oznacza spadek w porównaniu z rokiem 2016, kiedy całkowita liczba zgonów związanych z paleniem tytoniu wyniosła 3 868, a odsetek wszystkich zgonów przypisywanych paleniu tytoniu – 4,53%. W 2016 roku wskaźnik śmiertelności z powodu palenia tytoniu wynosił 5,5% wśród mężczyzn i 2,2% wśród kobiet. Dane te podkreślają nieustanny wpływ palenia tytoniu na stan zdrowia publicznego w Somalii, mimo spadku wskaźników śmiertelności w ciągu pięciu lat.
