En Bulgaria, fumar es legal con algunas restricciones. La venta de cigarrillos está permitida y su compra en máquinas expendedoras también es legal; sin embargo, no está permitido adquirirlos por internet. La importación de cigarrillos con fines comerciales es legal siempre que se cuente con la licencia correspondiente. Es obligatorio incluir advertencias sanitarias en los envases de los cigarrillos y la edad mínima legal para comprarlos es de 18 años. Además, existen restricciones legales sobre fumar en espacios públicos. Los cigarrillos están sujetos a una carga tributaria total del 85%. En 2023, había aproximadamente 2,1 millones de adultos fumadores en Bulgaria, lo que representa una prevalencia de tabaquismo del 37% en adultos. Entre los hombres, la prevalencia fue del 38,1% (datos de 2022) y entre las mujeres, del 30,2%. Ese mismo año, el número de fumadores diarios ascendía a 2,2 millones, con una prevalencia diaria del 34,4% en adultos; entre los hombres, la prevalencia diaria fue de entre el 34,4% y el 38% (datos de 2019) y entre las mujeres, del 21,1% (también de 2019). Según datos de 2021, el consumo de cigarrillos causó unas 14.100 muertes en Bulgaria, entre ellas 10.800 de hombres y 3.300 de mujeres, lo que equivale al 8,36% de todas las muertes del país, incluyendo el 12,35% de los fallecimientos masculinos y el 4,06% de los femeninos.
