W Republice Dominikany palenie tytoniu jest legalne, jednak obowiązują określone ograniczenia. Sprzedaż papierosów jest dozwolona, a ich zakup w automatach vendingowych jest również możliwy. Legalny jest także zakup papierosów online oraz ich import na potrzeby handlu, pod warunkiem posiadania licencji importowej. Na opakowaniach papierosów obowiązkowo muszą znajdować się ostrzeżenia zdrowotne, a minimalny wiek uprawniający do zakupu to 18 lat. Istnieją również przepisy ograniczające palenie w niektórych miejscach publicznych, takich jak placówki edukacyjne i ochrony zdrowia. Papierosy objęte są łączną stawką podatku w wysokości 0,45. Jeśli chodzi o rozpowszechnienie palenia, w 2022 roku odsetek dorosłych osób aktualnie palących wynosił 9,1%—12,5% wśród mężczyzn i 5,7% wśród kobiet. W 2021 roku oszacowano, że palenie tytoniu było przyczyną około 6 400 zgonów, w tym 4 100 wśród mężczyzn i 2 200 wśród kobiet. Oznacza to, że palenie odpowiadało za około 8,72% wszystkich zgonów w kraju—9,61% wśród mężczyzn i 7,45% wśród kobiet w 2021 roku.
