En Ecuador, fumar tabaco es legal, pero está sujeto a ciertas restricciones. La venta de cigarrillos está permitida, aunque no se pueden adquirir en máquinas expendedoras. Comprar cigarrillos por internet e importarlos para su comercialización es legal siempre que se disponga de una licencia. Además, es obligatorio que los paquetes de cigarrillos incluyan advertencias sanitarias, y la edad mínima legal para comprarlos es de 18 años. También existen restricciones legales para fumar en espacios públicos y la publicidad de cigarrillos está regulada. Los cigarrillos tienen una carga impositiva total del 64%. Según datos de 2022, Ecuador contaba con aproximadamente 1,3 millones de fumadores actuales, lo que corresponde a una prevalencia de consumo de tabaco en adultos del 10,1%. Entre los hombres adultos, la prevalencia era del 17,6%, mientras que entre mujeres adultas era del 2,6%. En 2021, se estimó que 3.600 muertes fueron atribuibles al consumo de tabaco, incluidas 2.800 en hombres y 808 en mujeres. Esto indica que el tabaquismo representó aproximadamente el 2,9% de todas las muertes en el país—un 3,94% en hombres y un 1,52% en mujeres durante ese año.
