In Sierra Leone, smoking is allowed but certain regulatory restrictions are applied on tobacco products including cigarettes. In line with these regulatory frameworks, cigarette sales are allowed, and consumers are permitted to purchase them online as well as through vending machines. However, importation of cigarettes for commercial trade requires an appropriate license. Notably, the government does not mandate health warnings on cigarette packaging, and there are currently no minimum legal age restrictions on sales. Furthermore, no legal restrictions exist for smoking in public places, and there is no comprehensive ban on all forms of tobacco advertising.

The current prevalence of tobacco smoking among adults aged 15 years and older in Sierra Leone was estimated to be 10% in 2024, with a significant gender disparity: 15.2% of males and 4.8% of females were current smokers. In other words, the total number of tobacco smokers in the country was approximately 524,000 that year.

Data from 2021 indicate that tobacco smoking resulted in 1,964 deaths in Sierra Leone, accounting for 2.48% of all deaths in the country. Among these, 1,437 were male deaths, corresponding to 3.36% of all male deaths, and 528 were female deaths, making up 1.45% of all female deaths. These figures highlight the ongoing public health issue caused by tobacco smoking in Sierra Leone, despite a reduction in smoking prevalence and associated deaths.
