В Островах Кука курение является легальным и регулируется с определёнными ограничениями. Согласно действующим правилам, продажа сигарет разрешена, однако их покупка через торговые автоматы или онлайн-запрещена. Импорт сигарет для торговли допускается при наличии соответствующей лицензии. На упаковке должны размещаться предупреждения о вреде для здоровья, а минимальный возраст для покупки сигарет составляет 18 лет. Также действуют законодательные ограничения на курение в общественных местах. Реклама сигарет полностью запрещена, а общий уровень налогообложения сигарет составляет 73%. По состоянию на 2024 год доля взрослых, которые курят на данный момент, составляла 26,2% — 29,7% среди мужчин и 23% среди женщин. В 2021 году насчитывалось примерно 3 000 человек, курящих ежедневно, при показателе ежедневного курения среди взрослых 18,8%: 23,3% среди мужчин и 14,6% среди женщин. В 2021 году курение сигарет привело по оценкам к 20 случаям смерти в Островах Кука, в том числе среди мужчин — 15, среди женщин — 5. Таким образом, курение стало причиной 12,17% всех смертей в стране: 15,76% среди мужчин и 6,96% среди женщин.
