Na Wyspach Cooka palenie tytoniu jest legalne, ale objęte pewnymi ograniczeniami. Sprzedaż papierosów jest dozwolona, natomiast kupowanie ich w automatach lub przez internet jest nielegalne. Import papierosów w celach handlowych jest dopuszczalny pod warunkiem posiadania odpowiedniej licencji. Opakowania muszą zawierać ostrzeżenia zdrowotne, a minimalny wiek uprawniający do zakupu papierosów wynosi 18 lat. Obowiązują również prawne ograniczenia dotyczące palenia w miejscach publicznych. Reklama papierosów jest całkowicie zakazana, a łączna stawka podatku na papierosy wynosi 73%. Według danych z 2022 roku częstość palenia wśród dorosłych na Wyspach Cooka wynosiła 25,5%, przy czym odsetek palących mężczyzn wynosił 30,3%, a kobiet – 21,4%. W 2021 roku liczba codziennych palaczy szacowana była na około 3 000 osób, a udział dorosłych palących codziennie wynosił 18,8% – 23,3% wśród mężczyzn i 14,6% wśród kobiet. W 2021 roku palenie papierosów było przyczyną szacunkowo 20 zgonów na Wyspach Cooka, w tym 15 wśród mężczyzn i 5 wśród kobiet. Oznacza to, że palenie odpowiadało za 12,17% wszystkich zgonów w kraju – 15,76% zgonów mężczyzn i 6,96% zgonów kobiet.
