En las Islas Cook, fumar es legal pero está regulado con ciertas restricciones. La venta de cigarrillos está permitida; sin embargo, no es legal comprarlos en máquinas expendedoras ni por internet. La importación de cigarrillos con fines comerciales está autorizada siempre que se cuente con la licencia correspondiente. Los empaques deben incluir advertencias sanitarias, y la edad mínima legal para comprar cigarrillos es de 18 años. También existen restricciones legales sobre fumar en espacios públicos. La publicidad de cigarrillos está completamente prohibida, y la carga tributaria total sobre los cigarrillos es del 73%. En 2022, la prevalencia de consumo de tabaco en adultos en las Islas Cook fue del 25,5%, con un 30,3% de hombres y un 21,4% de mujeres identificados como fumadores actuales. En 2021, había aproximadamente 3.000 fumadores diarios, con una prevalencia diaria de tabaquismo en adultos del 18,8% (23,3% en hombres y 14,6% en mujeres). Ese mismo año, se estimó que el consumo de cigarrillos causó 20 muertes en las Islas Cook, de las cuales 15 correspondieron a hombres y 5 a mujeres. Esto representa el 12,17% de todas las muertes en el país—el 15,76% de las muertes masculinas y el 6,96% de las femeninas.
