Palenie tytoniu jest dozwolone w Rumunii z określonymi ograniczeniami. Sprzedaż i import papierosów do obrotu są legalne, a konsumenci mogą również kupować je legalnie przez internet. Zabroniona jest jednak sprzedaż papierosów w automatach. Na opakowaniach obowiązkowo muszą znajdować się ostrzeżenia zdrowotne, a minimalny wiek uprawniający do zakupu wynosi 18 lat. Obowiązują także ograniczenia dotyczące palenia w miejscach publicznych. Szacuje się, że liczba obecnych palaczy wynosiła 5,4 miliona w 2023 roku. Wskaźnik palenia wśród dorosłych w 2023 roku wynosił 34%, w tym 17,5% wśród kobiet i 37,4% wśród mężczyzn w 2022 roku. Palenie tytoniu było przyczyną około 29 500 zgonów w 2021 roku, z czego 6 200 stanowiły zgony kobiet, a 23 300 mężczyzn. Oznacza to, że palenie odpowiadało za 8,82% wszystkich zgonów w tym roku—3,87% wśród kobiet i 13,38% wśród mężczyzn.
