W Republice Środkowoafrykańskiej brakuje danych na temat rozpowszechnienia palenia tytoniu. Jednak dostępne informacje wskazują, że śmiertelność związana z paleniem stanowi poważny problem zdrowia publicznego w tym kraju. W 2021 roku Global Burden of Disease (GBD) odnotowało 1 917 zgonów przypisywanych paleniu, w tym 281 kobiet i 1 636 mężczyzn. Szacuje się, że odsetek wszystkich zgonów spowodowanych paleniem tytoniu w 2021 roku wynosił 2,6%, przy czym w przypadku kobiet było to 0,89%, a wśród mężczyzn 3,88%. Dla porównania, w 2016 roku Tobacco Atlas odnotował wyższą całkowitą liczbę zgonów związanych z paleniem – 3 093, z czego 608 stanowiły kobiety, a 2 485 – mężczyźni. Odsetek zgonów przypisywanych paleniu w 2016 roku wynosił ogółem 5,05%, przy czym u kobiet 1,6%, a u mężczyzn 5,9%. Chociaż dane te wskazują na spadek zarówno bezwzględnej liczby zgonów związanych z paleniem, jak i ich odsetka wśród wszystkich zgonów w latach 2016–2021, obciążenie związane z paleniem pozostaje w kraju wysokie, dlatego konieczne są dodatkowe działania na rzecz ograniczania jego negatywnego wpływu na zdrowie publiczne.
