En el país se permite fumar tabaco, aunque existen ciertas restricciones. Los cigarrillos pueden venderse y comprarse legalmente, incluso por internet y en máquinas expendedoras. También se permite la importación con fines comerciales siempre que se cuente con la correspondiente licencia de importación. La comercialización de productos de tabaco está sujeta a limitaciones, pero no existe una prohibición total de la publicidad relacionada con el tabaco. Es obligatorio que los envases lleven advertencias sanitarias. No hay una edad mínima establecida para la compra de cigarrillos ni restricciones legales para fumar en lugares públicos. Los cigarrillos están gravados con una tasa impositiva total del 23,23%. En 2021, se estimó que 49 muertes en San Vicente y las Granadinas estuvieron relacionadas con el consumo de tabaco, de las cuales 38 correspondieron a hombres y 11 a mujeres. Esto implica que el tabaquismo fue responsable del 4,27% de todas las muertes en el país, representando el 5,64% de las muertes masculinas y el 2,28% de las femeninas en 2021.
