En Costa Rica, fumar tabaco es legal, pero existen ciertas restricciones. La venta de cigarrillos está permitida; sin embargo, no se permite comprarlos por internet ni en máquinas expendedoras. La importación de cigarrillos para su comercialización está autorizada con la debida licencia. Es obligatorio incluir advertencias sanitarias en los empaques de cigarrillos y la edad mínima legal para su compra es de 18 años. Además, hay restricciones legales para fumar en espacios públicos y está vigente una prohibición total de la publicidad de cigarrillos. Los cigarrillos están gravados con un impuesto total del 73%. En 2022, había aproximadamente 3,000 adultos fumadores diarios en Costa Rica, lo que equivale a una prevalencia de consumo diario en adultos del 18.8%. Entre los hombres, la prevalencia diaria fue del 23.3% y entre las mujeres, del 14.6%. La prevalencia de consumo actual entre adultos fue del 25.5%, con un 30.3% en hombres y un 21.4% en mujeres. En 2021, se estimó que 20 muertes estuvieron relacionadas con el consumo de tabaco, de las cuales 15 correspondieron a hombres y 5 a mujeres. Esto indica que el tabaquismo representó aproximadamente el 12.17% de todas las muertes en el país, con un 15.76% entre los hombres y un 6.96% entre las mujeres.
