In Antigua and Barbuda, smoking is legal and tobacco products are regulated. Cigarettes are legally sold and their importation for trade is permitted with a license. However, cigarettes cannot be purchased online or through vending machines. Health warnings on cigarette packaging are required, and there are legal restrictions on smoking in public places. All forms of tobacco advertising are banned. Cigarettes are not subject to a specific excise tax, but to a total taxation rate of 15%.

As of 2024, Antigua and Barbuda had an estimated 9,000 current adult smokers. In 2021, smoking caused an estimated 31 deaths, including 23 male deaths and 8 female deaths. Overall, smoking constituted 4.34% of all deaths in the country, including 5.93% of male deaths and 2.49% of female deaths.
