W Republice Południowej Afryki w 2021 roku było około 920 000 użytkowników e-papierosów, przy odsetku dorosłych korzystających z tych urządzeń na poziomie 2,2% — 0,7% wśród kobiet i 3,8% wśród mężczyzn. Dane te wskazują, że e-papierosy są dostępne w kraju, choć nie wiadomo, jak powszechne jest ich użytkowanie. E-papierosy są dozwolone i regulowane jako produkty lecznicze; można je nabyć w aptekach na receptę, natomiast zakupy online na własny użytek nie są dozwolone. Sprzedaż nikotyny jest legalna, jednak wymagane jest umieszczanie ostrzeżeń zdrowotnych na opakowaniach oraz obowiązują ograniczenia wiekowe — e-papierosy mogą kupować wyłącznie osoby powyżej 18. roku życia. Ponadto korzystanie z e-papierosów jest ograniczone w miejscach publicznych.
