W Bośni i Hercegowinie palenie tytoniu jest legalne, jednak obowiązują określone ograniczenia. Sprzedaż papierosów jest dozwolona, a ich zakup jest możliwy zarówno przez internet, jak i w automatach. Import papierosów w celu dalszej odsprzedaży jest również dozwolony, pod warunkiem posiadania odpowiedniej licencji importowej. Reklama produktów tytoniowych nie została całkowicie zakazana. Na opakowaniach papierosów nie ma wymogu umieszczania ostrzeżeń zdrowotnych, a prawo nie określa minimalnego wieku, od którego można kupować wyroby tytoniowe. Ponadto nie obowiązują żadne ograniczenia dotyczące palenia w miejscach publicznych. Na papierosy nakładany jest jednak łączny podatek w wysokości 84%. W 2022 roku w Bośni i Hercegowinie szacowano liczbę dorosłych palaczy na 968,5 tysiąca, co odpowiada rozpowszechnieniu palenia wśród dorosłych na poziomie 35,1%. Wśród mężczyzn odsetek ten wynosił 41%, a wśród kobiet 29,5%. W tym samym roku liczba osób palących codziennie wynosiła około 1 miliona, a odsetek dorosłych palących codziennie sięgał 32%. Wśród mężczyzn odsetek codziennych palaczy wynosił 40,2%, a wśród kobiet 23,9%. Dane z 2021 roku wskazują, że palenie tytoniu przyczyniło się do 6700 zgonów, w tym 4900 wśród mężczyzn i 1800 wśród kobiet. Oznacza to, że palenie tytoniu odpowiadało za 14,42% wszystkich zgonów w kraju—20,16% zgonów mężczyzn oraz 8,16% zgonów kobiet w 2021 roku.
