W Bułgarii palenie tytoniu jest legalne, jednak obowiązują pewne ograniczenia. Sprzedaż papierosów jest dozwolona, podobnie jak ich zakup w automatach, natomiast kupowanie papierosów przez internet jest zabronione. Import papierosów na potrzeby handlu jest możliwy tylko z odpowiednią licencją. Na opakowaniach papierosów muszą znajdować się ostrzeżenia zdrowotne, a minimalny wiek uprawniający do zakupu papierosów to 18 lat. Istnieją także prawne ograniczenia dotyczące palenia w miejscach publicznych. Na papierosy nakładany jest łączny podatek w wysokości 85%. Według danych z 2023 roku w Bułgarii było około 2,1 miliona dorosłych osób aktualnie palących, co stanowi 37% dorosłej populacji. U mężczyzn wskaźnik rozpowszechnienia palenia wynosił 38,1% (dane z 2022 roku), a u kobiet 30,2%. W tym samym roku liczba codziennych palaczy wyniosła 2,2 miliona, a udział dorosłych palących codziennie w populacji stanowił 34,4%—38% wśród mężczyzn (dane z 2019 roku) oraz 21,1% wśród kobiet (również 2019 rok). Według danych z 2021 roku palenie papierosów było przyczyną szacunkowo 14 100 zgonów w Bułgarii, w tym 10 800 wśród mężczyzn i 3 300 wśród kobiet. Oznacza to, że palenie odpowiadało za 8,36% wszystkich zgonów w kraju, w tym za 12,35% zgonów mężczyzn i 4,06% zgonów kobiet.
