En Bosnia y Herzegovina, fumar es legal con ciertas restricciones. La venta de cigarrillos está permitida, así como la compra tanto en línea como a través de máquinas expendedoras. La importación de cigarrillos para su comercialización también está autorizada siempre que se cuente con una licencia de importación. No existe una prohibición total de la publicidad. No es obligatorio incluir advertencias sanitarias en los paquetes de cigarrillos y no hay una edad mínima legal para la compra de productos de tabaco. Además, no existen restricciones para fumar en lugares públicos. Sin embargo, los cigarrillos están gravados con una tasa impositiva total del 84%. En 2022, se estimaba que Bosnia y Herzegovina contaba con 968.500 adultos fumadores actuales, lo que equivale a una prevalencia del tabaquismo del 35,1% entre adultos. Entre los hombres, la prevalencia era del 41%, y entre las mujeres, del 29,5%. Ese mismo año, había aproximadamente 1 millón de fumadores diarios, con una prevalencia diaria en adultos del 32%. Entre los hombres, la prevalencia diaria era del 40,2%, y entre las mujeres, del 23,9%. Los datos de 2021 indican que el consumo de tabaco provocó 6.700 muertes, de las cuales 4.900 correspondieron a hombres y 1.800 a mujeres. Esto significa que el tabaquismo fue responsable del 14,42% de todas las muertes en el país—el 20,16% de las muertes masculinas y el 8,16% de las muertes femeninas en 2021.
