Los cigarrillos electrónicos son legales en Bélgica bajo un marco regulatorio específico. Su venta y uso están permitidos, pero las ventas en línea están prohibidas. Aunque se permiten afirmaciones sobre la salud, toda publicidad está prohibida. Los cigarrillos electrónicos pueden adquirirse en comercios generales y no están sujetos a impuestos especiales. La normativa exige la notificación de nuevos productos, envases a prueba de manipulación infantil y de alteraciones, y advertencias sanitarias en el empaquetado. Además, la venta está restringida a personas mayores de 18 años y existen limitaciones legales para su uso en espacios públicos. El contenido de nicotina está limitado a 20 mg/mL o menos, y la capacidad de los depósitos no puede superar los 2 mL. Sin embargo, no existen restricciones sobre la variedad de sabores. En cuanto a la prevalencia, en 2023 había aproximadamente 383,900 usuarios actuales de cigarrillos electrónicos en Bélgica, lo que representa una prevalencia de uso actual en adultos del 4%. La prevalencia era mayor entre los hombres (12%) que entre las mujeres (8%). En ese mismo año, la prevalencia diaria de vapeo en adultos fue del 1.2% en general. En 2021, esta cifra era del 3% tanto en hombres como en mujeres. Como referencia, en 2017, el 16% de los adultos reportó haber probado alguna vez los cigarrillos electrónicos. Una encuesta realizada ese mismo año mostró que el 62% de los encuestados en Bélgica consideraba que los cigarrillos electrónicos son perjudiciales o más perjudiciales que los cigarrillos convencionales, el 34% opinaba que no son perjudiciales o son menos perjudiciales, y el 4% no estaba seguro.
