Курение по-прежнему остаётся одной из основных проблем общественного здоровья в Северной Македонии. По оценкам, в 2024 году в стране насчитывалось около 589 000 человек, которые курят в настоящее время, а распространённость курения среди взрослых составила 39,9%. При этом уровень курения среди мужчин был значительно выше (43,4%), чем среди женщин (36,7%). В 2021 году ежегодное количество случаев смерти, связанных с курением, оценивалось в 4 100, включая 2 800 среди мужчин и 1 300 среди женщин. На смертность, обусловленную курением, приходилось 12,66% всех случаев смерти в стране: 15,9% среди мужчин и 8,75% среди женщин. Борьба с курением в Северной Македонии осуществляется с помощью ряда нормативных мер: полностью запрещена вся реклама табачных изделий, продажа и употребление сигарет разрешены, включая возможность покупки онлайн, однако приобретение сигарет через автоматы запрещено. Также на упаковке обязательно размещаются предупреждения о вреде для здоровья, минимальный возраст для приобретения сигарет установлен на уровне 18 лет, действуют ограничения на курение в общественных местах, а для сигарет применяется общий налоговый коэффициент 0,77.
