W Japonii sprzedaż e-papierosów jest zabroniona, a tego typu produkty są traktowane jako produkty lecznicze. Oznacza to, że nie mogą być legalnie sprzedawane ani dystrybuowane bez odpowiedniego zezwolenia farmaceutycznego. Sprzedaż internetowa e-papierosów zawierających nikotynę również nie jest dozwolona, a ich komercyjny import jest zakazany. Niektóre nieoficjalne źródła sugerują, że japońscy użytkownicy mogą mieć możliwość sprowadzenia na własny użytek e-papierosów z nikotyną w ilości odpowiadającej miesięcznemu zapotrzebowaniu, jednak informacji tych nie udało się niezależnie potwierdzić. Pomimo ograniczeń regulacyjnych, według danych z przeprowadzonych w 2017 roku badań ankietowych, około 2,1 miliona dorosłych mieszkańców Japonii korzystało wówczas z e-papierosów, a odsetek obecnych użytkowników wśród dorosłych szacowano na 1,9%. W tym samym roku 11,4% osób zadeklarowało, że przynajmniej raz w życiu spróbowało e-papierosa.
