El tabaquismo sigue siendo un importante problema de salud pública en Macedonia. En 2017, el país registró un estimado de 786,800 fumadores actuales, con una alta prevalencia de tabaquismo en adultos del 46%. La prevalencia en adultos fue notablemente mayor en hombres (54.1%) en comparación con las mujeres (33.3%). En 2021, se estimó que el número anual de muertes atribuibles al tabaquismo fue de 4,100, incluidas 2,800 entre hombres y 1,300 entre mujeres. Se informó que la mortalidad por tabaquismo representó el 12.66% de todas las muertes en Macedonia, siendo del 15.9% entre los hombres y del 8.75% entre las mujeres. Los esfuerzos para controlar el consumo de tabaco en Macedonia se implementan mediante varias medidas regulatorias. Existe una prohibición total de toda publicidad de tabaco. La venta y el consumo de cigarrillos están permitidos, incluida la compra en línea; sin embargo, la adquisición a través de máquinas expendedoras está prohibida. Además, es obligatorio que los paquetes lleven advertencias sanitarias y la edad mínima legal para comprar cigarrillos es de 18 años. También existen restricciones legales para fumar en lugares públicos y se aplica una tasa total de impuestos de 0.77 a los cigarrillos.
