W Korei Północnej palenie tytoniu pozostaje poważnym problemem zdrowia publicznego. Według danych z 2022 roku, około 3 511 018 osób w wieku 15 lat i starszych paliło papierosy. Liczba ta oznacza niewielki spadek w porównaniu z 2020 rokiem, kiedy odnotowano 3 573 389 palaczy. Ogólna częstość palenia tytoniu wśród dorosłych w 2022 roku wyniosła 17,1%, co stanowi minimalny spadek w stosunku do 17,6% w 2020 roku. Wśród dorosłych mężczyzn odsetek ten wyniósł 34,7% w 2022 roku, podczas gdy w 2020 roku było to 36,4%. Dla porównania, w 2017 roku codzienne palenie wśród dorosłych wyniosło 18,3%, przy wskaźniku 38,4% wśród mężczyzn; nie odnotowano takich danych dla kobiet. W 2021 roku około 42 321 zgonów przypisano paleniu tytoniu, co stanowiło 17,48% wszystkich zgonów w kraju. Według danych z 2021 roku, wśród mężczyzn palenie tytoniu odpowiadało za 27,4% wszystkich zgonów, natomiast wśród kobiet za 8,13%.
